Looking for Sherit Hapletah

I was inscribed in the bar mitzvah book of the Sherit Hapletah when I had my bar mitzvah. I now wish to inscribe my sons in the same book but am unable to locate the Sherit Hapletah. I remember that the Sherit Hapletah certificate came from Israel.
